Citicoline and brain health

Citicoline is the generic name for cytidine-5′-diphosphocholine (CDP-choline), a compound chemically identical to the form naturally produced by the body to synthesize phosphatidylcholine, a key structural component of cell membranes.

Citicoline has been used in clinical medicine since the 1970s for its neuroprotective properties, particularly in conditions such as stroke, traumatic brain and spinal cord injury, Parkinson’s disease, and glaucoma. It is also available as a dietary supplement in the US and EU.

After ingestion, citicoline is hydrolyzed into cytidine and choline, which are taken up by brain cells to support membrane repair, neurotransmitter production, and protect neurons from damage.

Phosphatidylcholine (PC) is essential for cell membrane integrity and repair, but levels decline with age. Citicoline provides substrates for CDP-choline resynthesis, slowing phospholipid breakdown and accelerating membrane repair.

Healthy membranes improve the function of receptors, active signaling molecules in the brain, as well as physical plasticity. Repairing and maintaining these membranes helps to preserve the physical infrastructure where memory encoding actually happens.

A 2021 study in older adults with age-related memory impairment found citicoline supplements (500 mg/day for 12 weeks) improved overall memory, particularly episodic memory.¹ A 2022 review of over 800 studies concluded citicoline improves memory and attention in healthy people and those with vascular dementia.²

Citicoline has demonstrated neuroprotective effects in acute stroke by reducing glutamate excitotoxicity, oxidative stress, apoptosis, and blood-brain barrier dysfunction.³

During post-stroke recovery, citicoline appears to support neurorepair mechanisms such as neurogenesis, synaptic remodelling, and restoration of neurotransmitter function. A 2025 network meta-analysis found that stroke patients treated with citicoline achieved significantly higher functional scores at 1 and 3 months post-stroke compared with standard care alone.⁴

Citicoline has been shown to reduce pro-inflammatory cytokine production and mitigate oxidative stress in the brain. It enhances the activity of endogenous antioxidant enzymes, including superoxide dismutase (SOD) and glutathione peroxidase (GPx), potentially limiting inflammatory damage. It also improves the activity of microglia (immune cells of the central nervous system), helping to prevent excessive neuroinflammatory responses that contribute to neuronal injury. 

A 2024 review found citicoline decreased pro-inflammatory cytokines in Alzheimer's disease models, and it was thought to help manage Parkinson's disease by enhancing dopamine levels and reducing neuroinflammation.⁵

Citicoline is naturally present in the human body and has been used in medicine for decades of medical use without any significant adverse effects.

Most studies involving several months of daily supplementation have reported it to be safe and well-tolerated. Future investigations are required to determine the acute and longer-term effects.⁶

Most clinical studies involving citicoline supplementation have used doses ranging from 500-1,000 mg per day. Doses around 500 mg/day have been given to patients recovering from stroke, while higher doses (up to 1,000 mg/day) are more often used in cognitive and neuroprotective studies. 

Dosage should be individualized and discussed with a qualified healthcare professional.

Choline is a vitamin-like nutrient, while citicoline is choline bonded to cytidine via a pyrophosphate group. 

Upon ingestion, citicoline is broken down in the intestines into choline and cytidine, which then enter the bloodstream separately. While standalone choline supplements provide choline alone, citicoline additionally supplies cytidine, which may enhance phospholipid synthesis and neuronal membrane repair. Citicoline exhibits high oral bioavailability, with minimal fecal excretion reported in human studies.⁷

Most forms of choline have lower bioavailability, with choline bitartrate the lowest.

Unlike some choline salts, citicoline is not typically associated with symptoms of cholinergic toxicity at high doses. 

Citicoline is not banned, but regulation varies between countries. It is available over the counter as a dietary supplement in the US and UK under brand names such as Cognizin®. It may require a prescription in some countries in Europe and Asia.

Energy drinks made with citicoline and caffeine are widely available and are marketed to improve cognitive performance, mood, and energy levels. A 2024 study reported that a widely available brand of energy drink containing citicoline helped to improve cognitive performance, visuospatial gaming performance, and mood, but had no effect on energy expenditure.⁸ Further research is required to establish the benefits of these drinks.

Pregnant or breastfeeding women are advised to avoid citicoline due to insufficient safety data in these populations.

Individuals with medical conditions or those taking medications such as levodopa, cholinergic agents, or anticoagulant therapies should seek medical advice prior to supplementation.

Citicoline is not recommended for children unless under medical supervision.
