What is Leucovorin?
Leucovorin, also known as folinic acid, is a prescription form of reduced folate closely related to vitamin B9. Unlike synthetic folic acid, leucovorin is already a reduced folate form capable of participating in several folate-dependent biochemical pathways without requiring the full series of enzymatic conversion steps normally needed for standard folic acid metabolism.1
In clinical medicine, leucovorin is primarily used to “rescue” healthy cells from the effects of certain medications that interfere with folate metabolism, particularly methotrexate. For this reason, leucovorin is often referred to as “leucovorin rescue” in oncology and hospital settings. It is also commonly used alongside chemotherapy medications such as 5-fluorouracil (5-FU), where it helps enhance the therapeutic activity of the drug.2
Because folate supports biochemical pathways involved in DNA synthesis, cellular repair, methylation, homocysteine metabolism and neurotransmitter-related processes, leucovorin has become an important medical tool in situations involving impaired folate pathways or needing increased folate demand.
Leucovorin belongs to a group of compounds known as reduced folates (some might refer to these as “activated folate forms”). This distinction is important because not all forms of vitamin B9 function the same way in the body. Folate metabolism is complex and involves multiple conversion steps before folate can participate in cellular processes. Different forms of folate - including folic acid, folinic acid (leucovorin) and L-methylfolate - enter this pathway at different stages and may affect the body differently.
Unlike folic acid, leucovorin does not require conversion by the dihydrofolate reductase (DHFR) enzyme. However, leucovorin is also not identical to methylfolate, the biologically active methylated form of folate directly involved in methylation and homocysteine recycling. Leucovorin cannot bypass the MTHFR gene like L-methylfolate does. These distinctions have become increasingly important in discussions involving folate metabolism, MTHFR variants, neurological health and personalized nutrition.
Today, leucovorin is widely recognized as both a pharmaceutical folate therapy and an important part of the broader scientific conversation surrounding folate metabolism, methylation and cellular health.3
