How Does Leucovorin Work for Autism?
Leucovorin, also known as folinic acid, is a reduced and biologically active form of folate. Unlike folic acid, leucovorin does not require extensive enzymatic conversion before it can participate in important cellular processes, including DNA synthesis, methylation, and neurotransmitter production.1
Interest in leucovorin for autism emerged following the discovery that some children with ASD have impaired folate transport to the brain, which may affect neurodevelopment and brain function. One of the most well-studied mechanisms involves folate receptor alpha autoantibodies (FRAAs), which can interfere with folate transport across the blood-brain barrier.2
When folate delivery to the brain is impaired, a condition known as cerebral folate deficiency (CFD) may develop. Folate plays a critical role in brain development and function, including the production of neurotransmitters, regulation of gene expression, and support of neurodevelopmental processes.3
Leucovorin may help by bypassing some of the transport limitations associated with FRAAs and increasing the availability of folate within the central nervous system. This has led researchers to investigate leucovorin as a potential supportive therapy for certain individuals with autism, particularly those with evidence of cerebral folate deficiency or folate receptor autoantibodies.4
Several clinical studies have reported improvements in language, communication, attention, and social interaction among some children receiving leucovorin for autism treatment. Researchers believe these effects may result from improved folate-dependent metabolic processes within the brain, although the exact mechanisms continue to be investigated.5
It is important to note that leucovorin is not considered a cure for autism. Rather, current research suggests it may provide benefits for specific subgroups of individuals with ASD, especially those with underlying folate-related abnormalities.6
